While tooth decay is a potential problem at any age, the risk increases as you grow older. Not only are senior adults more susceptible, decay is often more difficult to treat. That’s because cavities can occur at or below the gum line, often due to gum recession.
If that’s the case, we won’t be able to fill the cavity in the usual way because the gum tissue is in the way. To make it possible to treat, we may need to perform a minor surgical procedure known as crown lengthening.
As the name implies, the procedure helps expose more of the crown, the tooth’s visible part, from the surrounding gum tissue. In basic terms, we’re repositioning the gum tissue away from the biting surface of a tooth to make room for a filling. It’s also useful for improving a tooth’s appearance by making it look longer, or creating room for a crown or other dental restoration.
After applying a local anesthesia, the dentist (or periodontist, a specialist in the gums) creates a tiny flap of gum tissue with a series of incisions. This allows the dentist to move the affected gums out of the way to access the underlying bone. The dentist then reshapes the bone to adequately support the gum tissue when it’s finally repositioned to expose the crown. In its new and improved position, the dentist sutures the gum tissue in place.
For a few days afterward, the patient will need to restrict their diet to soft foods, avoid strenuous activities and apply an ice pack to help reduce swelling the day of surgery. They will also prescribe a pain reliever and possibly an antibiotic to reduce the chances of infection.
While most people return to normal activities a few days later, you’ll usually have to wait a few weeks for the gums to heal before undergoing any further treatments for the affected teeth. But even with the wait, crown lengthening could make it possible to not only save your tooth but improve your smile as well.

If you want to keep that new smile after orthodontic treatment, you’ll need to wear a retainer for awhile. Teeth have a tendency to “rebound” to their old positions and a retainer prevents that from happening.
Most people are familiar with the standard removable retainer. But there’s another option: a bonded retainer. While performing the same function as a removable one, the bonded retainer differs in one important aspect—it’s fixed in place and can’t be removed except by a dentist. It’s especially useful for certain bite repairs like the closure of the gap between the front teeth.
If you’re thinking this retainer sounds a lot like the braces just removed, it’s not. The main part of a bonded retainer is a thin metal wire that we bond with a dental composite material across the back of the affected teeth. While you can definitely feel it with your tongue it can’t be seen by others, which is an advantage over many removable retainers.
The fixed nature of bonded retainers also creates a couple of advantages, especially for younger patients. There’s no compliance issue as with removable retainers—the patient doesn’t have the option of taking it out. That also means it can’t be lost, a frequent and costly occurrence with the removable variety.
But a bonded retainer does have some drawbacks. For one, the wire and composite material make it more difficult to floss. There’s also a possibility of breakage from high biting forces, which if that should occur must be immediately repaired to avoid the teeth rebounding. But while removable retainers have their downsides, it’s much easier with them to keep the teeth clean of plaque—you simply take the appliance out to brush and floss.
With your dentist’s help you can weigh the pros and cons of both types of retainers and decide which is best for you or your child. Whichever one you choose, wearing a retainer will help protect that hard-earned smile for years to come.

A relatively easy and inexpensive way to improve your smile is to whiten your teeth. In fact, you may even be able to do it yourself at home. Although there are differences between at-home and professional whitening, the former can still be effective.
That effectiveness, though, might be dependent on how well you understand what home whitening can do and what it can’t. So before you purchase your kit, here are a few things to know about DIY teeth whitening.
Home kits are safe—when used as directed. Although nothing in life is completely risk-free, home whitening kits are relatively safe. But only if you follow the directions: If you overuse the bleaching solution beyond its intended use, you could wind up with permanent damage to your tooth enamel that can be costly to treat.
Bleaching solutions are relatively mild. Nearly all home kits use carbamide peroxide, a form of hydrogen peroxide, as their bleaching agent. Home kits usually contain no more than 10% of this agent, much lower than professional solutions. Used as directed, this amount of bleaching agent poses little to no risk to dental or general health. On the other hand, the weaker home kit solution does limit the extent of brightness you can achieve compared to stronger professional treatments.
For top results (or certain conditions), consider professional whitening. While you can obtain good results with a home whitening kit, professional whitening provides more precise brightness control and range, and may possibly last longer. That’s because your dentist has more advanced techniques and equipment, as well as stronger bleaching solutions. And, if your discoloration originates inside your teeth, a home kit won’t help—only a dentist can address that kind of discoloration.
Even if you decide to “go solo,” your dentist can still give you advice on what to look for in a home kit, as well as tips for the actual procedure. But whether you whiten at home or let your dentist do it, teeth whitening can take years off your smile and give you a more attractive look.

You may be able to slow the aging process with healthy habits but you can’t stop it. Every part of your body will change, including your teeth and gums. And even with great dental hygiene and care, there are at least two aging outcomes you may not be able to avoid: discoloration and tooth wear.
Fortunately though, we have ways to counteract these effects and help you enjoy a much younger-looking smile. These techniques range in complexity and cost, but when tailored to your individual situation they can make a world of difference and restore your confidence in your smile.
Brightening teeth that have yellowed with age can be as simple as undergoing teeth whitening. The bleaching solution in this procedure (performed in the office or at home with a prescribed kit) can minimize enamel staining built up over the years. It can even be performed with some control over the level of desired brightness. Although whitening isn’t permanent, with proper care and regular touch-ups you can keep your youthful, dazzling smile for some time.
Tooth whitening, however, may not be enough in some cases of discoloration. If so, you can gain a bright new smile with porcelain veneers or crowns. A veneer is a thin layer of tooth-colored material bonded to the front of a tooth; a porcelain crown completely covers a tooth and is usually cemented onto it.
Normal tooth wearing can also affect the appearance of older teeth, making them look shorter and with less rounded edges than younger teeth. Veneers and crowns can be utilized for this problem too, as well as enamel shaping with a dental drill to minimize those sharp edges and project a softer, younger appearance. In extreme cases, surgically reshaping the gums can give teeth a longer and a more natural look.

Orthodontics shares a principle with the classic tug of war game: if you want things to move in the right direction you need a good anchor. Anchors help braces and other appliances apply constant pressure to misaligned teeth in the direction they need to go to correct a malocclusion (poor bite).
Orthodontic treatments work in cooperation with an existing oral mechanism that already moves teeth naturally in response to biting forces or other environmental factors. The key to this mechanism is an elastic tissue known as the periodontal ligament that lies between the tooth and the bone. Besides holding teeth in place through tiny attached fibers, the ligament also allows the teeth to move in tiny increments.
Braces’ wires laced through brackets affixed to the teeth exert pressure on them in the desired direction of movement –the periodontal ligament and other structures do the rest. To maintain that pressure we need to attach them to an “anchor”—in basic malocclusions that’s usually the back molar teeth.
But not all malocclusions are that simple. Some may require moving only certain teeth while not moving their neighbors. Younger patients’ jaws and facial structures still under development may also need to be considered during orthodontic treatment. That’s why orthodontists have other anchorage methods to address these possible complications.
One example of an alternate anchorage is a headgear appliance that actually uses the patient’s skull as the anchor. The headgear consists of a strap running around the back of the head and attached in front to orthodontic brackets (usually on molar teeth). The pressure it exerts can trigger tooth movement, but it can also help influence jaw development if an upper or lower jaw is growing too far forward or back.
Another useful anchorage method is a tiny metal screw called a temporary anchorage device (TAD) that is implanted into the jawbone above the teeth through the gums. Orthodontists then attach elastic bands between implanted TADs and specific braces’ brackets or wires to exert pressure on certain teeth but not others with pinpoint accuracy. After treatment the TADs can be easily removed.
Using these and other appliances allows orthodontists to customize treatment to an individual patient’s particular malocclusion. With the right anchor, even the most complex bite problem can be transformed into a beautiful and healthy smile.
